HOME Nurse Services is a provider of in-home specialist nursing, medical and home care. To help you live independently at home, our highly experienced Nurse Practitioners specialise in primary health and aged care, and will efficiently coordinate your home care package for you. Our service is ideal for people who wish to ensure that they, or their loved ones, receive the best of care in their home. In addition to treating illnesses, providing wound, catheter and post hospital discharge care, Home Nurse Services’ Nurse Practitioners can prescribe medication, make medical referrals and assist with the understanding, communication and management of necessary clinical care. Importantly, through active collaboration, Home Nurse Services complements the existing care being provided by general practitioners and other medical services providers. This approach can help clients proactively manage their health to improve quality of life, avoid undesirable acute issues and reduce hospitalisation.
