Location
Albany Community Care Centre is your local community aged care provider. Our services are highly regarded because we provide dedicated support staff ensuring you do not have multiple people coming through your home. Our defining principles: Respect: We respect your choices, your privacy, your home and your security. This is why we believe it is important to provide you with support staff that you feel safe with who are responsive to your needs. Relationship: As local people we know the importance of community and how reassuring it is to have a relationship with your support worker. When you call Albany Community Care our local coordinator answers the phone and is usually able to resolve any support requests you may have at the time of the call. Reliability: Our goal is to arrive at your home on time every time and if for some reason we will not be able to do this then we will call and arrange a new time that is convenient to you.

Home Care package level 2
Low-level care
Personal care, household or domestic assistance, social support, meals or shopping, or rehabilitation. Services may include loan of basic equipment.
$34,174.95 package
Home Care package level 3
Mid-level care
Personal care, household assistance, meals and medications, nursing or allied health input, support with memory or behaviour changes, or assistance with aids and appliances.
$51,808.10 package
Home Care package level 4
High-level care
Personal care, household assistance, meals and medications, nursing or allied health input, support with memory or behaviour changes, or assistance with aids and appliances.
