Location
Home Care Packages provided through Merri Health support people over 65, people with dementia, and people with disabilities to live in their homes and communities, for as long as possible. Our services help achieve personal goals – whatever they maybe. After a face-to-face meeting, we work with you to create a care plan that puts in place services or treatments to help you stay as healthy and independent as possible. • Our bi-lingual team includes specialists from a wide range of backgrounds including Nursing, Social Work and Nutritionist and we can assist you to create a package of care to suite your needs, as our Packages are tailored to your personal independence goals. We can help with: Service coordination, Personal care assistance, Gardening and home maintenance, Nursing support, Personal alarm support, In-home respite, Transport assistance, Support to access social and recreational activities, Carer Support and referrals to Allied health

Home Care package level 2
Low-level care
Personal care, household or domestic assistance, social support, meals or shopping, or rehabilitation. Services may include loan of basic equipment.
$39,310.50 package
Home Care package level 3
Mid-level care
Personal care, household assistance, meals and medications, nursing or allied health input, support with memory or behaviour changes, or assistance with aids and appliances.
$59,593.55 package
Home Care package level 4
High-level care
Personal care, household assistance, meals and medications, nursing or allied health input, support with memory or behaviour changes, or assistance with aids and appliances.
