Community Options is part of the Wimmera Health Care Group. It provides assistance to people and their carers to remain living independently in their own homes. Our range of services are co-ordinated and tailored to meet individual carer needs Community Options provides support to people who may be frail, aged, have chronic illness, who are palliative, living with dementia and mental health conditions. Community Options welcomes enquiries and referrals. Referrals can come from individuals, carers, health professionals, family or friends. We operate across Horsham Rural City Council, Hindmarsh Shire, West Wimmera Shire, Yarriambiack Shire & Buloke Shire.
