Country Health Connect know how important staying at home is for us all and we are able to structure support services around you or your loved one to make this possible. Whether you are recovering from an operation and need short term support or simply need some help to get out and about and stay connected to your community, Country Health Connect can help. We offer basic level support through our Home Support Program, and more complex care in the home though our Home Care Package program. We can tailor our service to meet your needs. Our professional and well-trained staff will work with you to find solutions to keep you well and independent at home. You won’t have to worry about coordinating or booking different services – simply talk to us and we will do the rest.

Home Care package level 2
Low-level care
Personal care, household or domestic assistance, social support, meals or shopping, or rehabilitation. Services may include loan of basic equipment.
$40,945.70 package
Home Care package level 3
Mid-level care
Personal care, household assistance, meals and medications, nursing or allied health input, support with memory or behaviour changes, or assistance with aids and appliances.
$62,075.55 package
Home Care package level 4
High-level care
Personal care, household assistance, meals and medications, nursing or allied health input, support with memory or behaviour changes, or assistance with aids and appliances.
