Focal’s Aged Care Services cater for those who have received approval from the Aged Care Assessment Team (ACAT) and a referral from My Aged Care. Focal supports the provision of both Low and High Home Care Packages. Focal will respond to your initial enquiry within 24 hours. Upon discussing you and your loved ones’ needs, wants and goals, we will present your personalised care plan within three working days. You and your loved ones will then have the freedom to choose the supports that you require, as well as your support worker. As you receive your services, Focal will continue looking for ways to improve them. We can schedule routine need assessments, or do this when it suits you and your loved ones.
