Our experienced and friendly Care Consultants will assist you to design the services you are looking for and how they are managed. Home Care Packages are tailored to the type of support you require. Therefore they can cover a wide range of services from clinical care, personal services and support in and around your home, to support within your community. How does Fronditha Care become your Home Care Package provider? Just let the assessor from the Aged Care Assessment Team or My Aged Care know you would like Fronditha Care to be your provider. Alternatively, once you have been assigned a package you can contact us directly.

$15,705.95 package
Home Care package level 2
Low-level care
Personal care, household or domestic assistance, social support, meals or shopping, or rehabilitation. Services may include loan of basic equipment.
$34,174.95 package
Home Care package level 3
Mid-level care
Personal care, household assistance, meals and medications, nursing or allied health input, support with memory or behaviour changes, or assistance with aids and appliances.
$51,808.10 package
Home Care package level 4
High-level care
Personal care, household assistance, meals and medications, nursing or allied health input, support with memory or behaviour changes, or assistance with aids and appliances.
